Account Manager – CarepathRx Join CarepathRx as an Account Manager, where you will help patients transition safely and confidently from hospital care to home-based therapy. In this role, you will build strong referral relationships, coordinate complex home-care services, and partner with clinical and operational teams to deliver seamless, cost-effective care and a positive patient experience. Responsibilities Provides a single point of contact for Physician offices while building relationships to promote specialty infusions for patients at home. Promotes and maintains the continuum of patient care from the hospital to home setting specific to patients in need of Home IV therapy and Enteral Nutrition. Build and maintain strong relationships with assigned hospitals, clinics, physician offices, home health agencies, and community partners. Collaborate with multidisciplinary teams to coordinate quality, cost-effective services in the home and support positive patient outcomes. Manage the referral process from initial intake through service coordination, including insurance verification, home health scheduling, and communication of financial responsibilities. Partner closely with operational teams to ensure timely referral processing, service readiness, and a smooth patient transition. Provide appropriate basic education to patients and families regarding therapy and next steps in the care process. Identify patient, family, or referral-source concerns and coordinate solutions that protect service quality and satisfaction. Support territory growth by engaging referral sources and communicating the value of CarepathRx services. Maintain accurate documentation and protect the confidentiality of patient information and records. Participate in required education, departmental meetings, and training to strengthen knowledge and service delivery. Required Qualifications Minimum 3 years of relevant experience in healthcare, account management, referral coordination, insurance, patient services, or a related customer-facing role. Strong project management, organizational, analytical, and problem-solving skills. Excellent written, verbal, interpersonal, and customer-service communication skills. Ability to manage multiple priorities and work independently in a fast-paced environment. Proficiency with Microsoft Office tools, including Word and Excel. Ability to collaborate effectively with hospital staff, physicians, home health agencies, community resources, patients, and families. Demonstrated ability to identify customer needs and develop practical, timely solutions. Preferred Qualifications Bachelor’s degree or equivalent professional experience. Experience in managed care, insurance, home infusion, specialty pharmacy, or home-based patient services. Public speaking, provider education, or referral-source presentation experience. Experience supporting patient transitions from acute-care settings to home-based therapy. If you will be working at home occasionally or permanently, the internet connection must be obtained through a cable broadband or fiber optic internet service provider with speeds of at least 10Mbps download/5Mbps upload. This role is also anticipated to be eligible to participate in an incentive compensation plan.
